The Wellness Assessment

How invigorated is your life ? Take this assessment to find out:

1. On average, how many servings of fruit do you typically eat a day?

2. On average, how many servings of vegatables do you typically eat a day?

3. How often do you eat legumes, beans, nuts or seeds in a week?

4. How often do you exercise each week? (minimum of 30 minutes at a time)

5. How many cups of coffee do you typically drink each day?

6. On average, how many hours of continuous sleep do you get each night?

7. How many cups of green tea do you drink a day?

8. On average, how many times a week do you eat out?

9. How many "diet" drinks do you consume in a week?

10. On average, how many times a week do you eat food with partially hydrogenated oil (trans fat) in it?

11. How often do you eat a meal from a box? (eg. Frozen dinners, Rice-a-Roni, Hamburger Helper, Mac'n'Cheese etc)

12. On average, How much water do you drink each day? (1 glass = 8 ounces)

13. How often do you take Juice Plus products?

14. How often do you practice meditation/ quiet time/ prayer in a given week?

15. How would you evaluate your stress level?



16. How do you view your own body image/overall health?
